Guidelines for Funding Requests

1.  Discuss your need with your CAM
2.  Visit GAL7Foundation.org website to identify other possible funding  and advise what other funding has been explored
3.  Get Funding Request Form from GAL7Foundation.org
4.  Fill out the form in specific detail of child’s needs (size, color, quantity, item#, etc.)
5.  Advise how (pickup/delivery), where and to whom item is to be delivered.
6.  Submit online for fast processing. Questions? email directly to Funding Chair [email protected]
7. Upon approval by the Funding Committee, you will receive funding and be responsible for providing receipts  (unless item is available & ordered by the Foundation through Wal-Mart)

All funding requests should be submitted through the website including requests for gas cards or food cards..

The funding committee is concentrating on activities impacting normalcy. These activities include camps especially during breaks in school; tutoring, sports teams, or after school activities.  Enrichment activities such as music lessons and art classes are  also supported.

Please be specific as to size, color, and type of clothing or shoes requested.

Use a complete description of the article requested and/or a link to a website or product at a store such as Walmart.

The Funding Committee reserves the right to contact CPC or other case managers to ensure all funding sources are used in fulfillment of a request.

If approved for purchase, be sure to save your receipts which will need to be submitted to the Foundation. Failure to submit receipts will result in funds being reported as the GAL's personal income.
